Can't create authentication module in IE 11 on Windows Server 2012

      Nothing happens when "create" is clicked to create an authentication module.

      Steps to reproduce:

      1. Set up vanilla Windows Server 2012 R2.
      2. Open internet explorer and connect to OpenAM 13.5
      3. top level realm > authentication > modules > add module
      4. Fill in a module name and select any module type. Nothing happens.

      Strangely, I have only managed to reproduce this on Windows Server 2012 R2. I have Windows 7 and 8.1 VMs that do not do this with fully patched versions of IE 11.

      No errors appear in the IE console during the entire course of the above steps (unlike related Jira OPENAM-7523).

      Note that the attached screenshot shows some warnings in the console. Also the icons don't appear to render correctly (font awesome?) but I found that putting IE in protected mode seems to fix that. It doesn't seem to fix this issue though.

      As the IE on Windows Server is shipped with stronger security defaults, I started removing these, but it did not make a difference.
