'Not Found' error in UI when opening a custom auth module created with ssoadm with the name the same as type

      When using the OpenAM 13 UI to open a custom authentication module that has been added via ssoadm where the name parameter is the same as the authtype parameter, a "Not Found" error is seen.

      To reproduce the currently observed behaviour:

      1. Follow the steps in the documentation to set up the Sample Authentication Module - https://backstage.forgerock.com/#!/docs/openam/13/dev-guide#sec-auth-spi but don't create the module using the UI (as in step 4.3.8).

      2. Add a module instance using the following ssoadm command (note that the module name and type are both set to SampleAuth):

      ./ssoadm create-auth-instance -u amadmin -f pwd.txt -e / --name SampleAuth --authtype SampleAuth

      3. Use the admin UI and go to Authentication > Modules and click the 'SampleAuth' link this results in a 'Not Found' error in the browser. The same behaviour is observed when trying to edit or delete the module.

      Expected behaviour:

      The module instance should be able to be opened, edited or deleted as expected.
