Cannot create a managed user with explicit mapping in a MySQL repo.

      Steps to reproduce:

      1. Configure OpenIDM with explicit table mapping with MySQL
      2. Try to create managed user, for example

      Expected result: User is correctly created.

      Actual result: Internal Server Error is observed.
